We provide Immediate Needs Assessments and case management for adults and children with acquired and traumatic brain injuries. We particularly work with complex care needs and challenging behaviours.
This could be clients with Cerebral Palsy, Learning Disabilities, Epilepsy, complications after Meningitis/Encephalitis, neurological conditions like Multiple Sclerosis, or after accidents including road traffic accidents.
We assess our clients’ needs, agree, organise and coordinate rehabilitation, all support and health and social care services to maximise their potential. We help them navigate the complex landscape of education, health and social care, acting as their advocate. We make things happen.

We initiate our journey by engaging with clients and their families to establish a strong foundation built on trust and understanding.
Our team conducts thorough Immediate Needs Assessments to determine the exact support package needed, including meticulous goal management. Ongoing assessments ensure that our clients’ evolving needs are met effectively.
Following assessments, our professionals provide detailed reports outlining the identified needs and proposed support packages. Transparency and communication are at the core of our reporting process.
We collaborate with NHS Professionals individually and in meetings. We arrange and accompany clients to health appointments, NHS or privately, supporting all our clients’ needs. We facilitate access to private clinical therapy input and collaborate with other professionals, ensuring a holistic approach to support.
We organise and chair regular Multi-Disciplinary Team (MDT) Meetings.
Four scheduled Multi-Agency/Multi-Disciplinary (MDT) meetings per year bring together all relevant parties to plan, monitor, and review clients’ goals, care packages and services. Extraordinary meetings are convened as needed for medical, educational, or other identified needs.
We actively participate in school reviews and education meetings, contributing valuable insights to support our clients’ educational needs.
Scheduled meetings with clients and their representatives occur on a monthly or bimonthly basis to ensure continuous monitoring, review, and adaptation of support to meet evolving goals and needs.
We provide assistance with health appointments, ensuring our clients receive the necessary care and attention.
Efficient management of care teams, from recruitment and selection to supervision, appraisal, and training, ensures the highest level of service. Disciplinary and appeal processes are handled with professionalism.
We take care of payroll setup and monthly systems, along with managing insurance cover to guarantee a seamless operational environment.
Beyond the outlined services, we remain flexible to address any additional needs that may arise, ensuring a tailored and client-focused approach.
